This is the central document of the formative assessment. It is used to verify if your project complies with the reference criteria and indicators. The questionnaire comprises four sub-pages, one for each Step of the assessment.
The questionnaire has two columns, the first for you, the project owner, and the second for the assessor to provide feedback. You, the project owner, must complete all four sections before the assessor starts.
