Instructions

Please read these instructions!

I. We will treat your answer anonymously and under no circumstances will we disclose your identity or share your personal information with a third party.

II. One problem, one form: for each problem you wish to report, you must complete a questionnaire.

III. This form has three sections:

  1. Section 1: Contact information
  2. Section 2: Problem analysis
  3. Section 3: Stakeholder analysis

IV. The questions with a red star (*) are mandatory.

V. Don't forget to click on the "Submit" button at the bottom of the page after you have filled out the form completely. 

VI. If you prefer to work offline, you can:

  3. Return to this page and copy and paste your answers.
  4. Don't forget to click on "Submit" at the bottom of the form.

How to write a problem statement.

A problem statement should describe an undesirable gap between the current-state level of performance and the desired future-state level of performance. A problem statement should include absolute or relative measures of the problem that quantify that gap, but should not include possible causes or solutions! Keep your statement short and clear. Two or three phrases should be enough.

Note, a missed opportunity is also a problem.
